“How can we support people to Self-Care?”

Health Education England had identified Integrated Care as one of its key priorities. HEE is committed to supporting the workforce to deliver health and care services that are both person centred and co-ordinated.

The West Midlands Integrated Care Transformation Theme recently launched a range of resources to support Integrated Care on the HEE West Midlands Learning Portal|.

A set of valuable tools and resources which can be used by care workers and service commissioners and many of these will also support unpaid carers and citizens. Additionally, we have identified valuableresources for carers| across social care through our work with Skills for Care. The resources are focused around dementia, end of life care, nutrition and hydration, manual handling and infection prevention and control.
